Montreal Lipedema

Living with lipedema means navigating a constant duality: the relentless physical discomfort and the hidden mental fatigue that follows.

Overview

Lipedema is a chronic condition characterized by the irregular and symmetrical accumulation of fat buildup, particularly in the lower body (in the legs, arms, and lower trunk). It is often mistaken for obesity or simple weight gain, but lipedema has distinct symptoms that set it apart:

Common Areas Affected by Lipedema
  • Legs: From the hips to the ankles, often sparing the feet, leading to a characteristic “cuff” appearance at the ankles.
  • Arms: Less commonly affected but can also display irregular fat accumulation.
  • Buttocks and Thighs: These areas are usually more prominently affected, contributing to a disproportionate body shape.

What is Lipedema?

Lipedema is a chronic disorder of the adipose tissue characterized by a symmetrical, disproportionate accumulation of fat—usually in the legs, hips, and sometimes the arms. Unlike common obesity, this tissue is often tender to the touch and typically resistant to traditional diet and exercise. If left unmanaged, the buildup can eventually compress the lymphatic system, leading to secondary fluid retention.

Estimates suggest that up to 11% of women (approximately 1 in 9) in Canada are affected by lipedema. Based on population estimates, this could mean as many as 2.2 million Canadians are living with this condition.

HOW IS LIPEDEMA CLASSIFIED?

Lipedema is classified by 1. STAGE (severity) and 2. TYPES (body sites affected).

1. Stages of Lipedema: (severity)

Lipedema is a progressive condition that worsens over time if left untreated. It is classified into different stages based on severity and types depending on which areas of the body are affected.

  • Stage 1:  Thickened skin with soft, small palpable nodules.  The skin surface is smooth
  • Stage 2:  Some larger nodules of fat, skin surface is uneven.
  • Stage 3:  Hardened large fat nodules prominent, with disfiguring fat distribution
Stages of Lipedema

2. Types of Lipedema:

Lipedema is categorized into five different types, based on the areas of the body affected.

  • Type I:  Buttocks and hips
  • Type II:  Thigh (Buttocks to knees)
  • Type III:  Thigh and lower legs
  • Type IV:  Arms
  • Type V:  Rare, with fat accumulation limited to the lower legs (Knees to Ankle).
Types of Lipedema

Key Symptoms of Lipedema:

Lipedema may be found in women of all shapes, sizes, ages and ethnicities. Lipedema presents with a number of symptoms - the below is not an exhaustive list, and not all patients will display all symptoms.

  • Disproportionate Fat Distribution: Fat accumulates in a symmetrical pattern on the legs, buttocks, hips, and sometimes arms, while the hands and feet are typically unaffected, creating a "cuff" or "bracelet" effect around the ankles and wrists.
  • Sensitivity and Heaviness: Affected limbs often feel hypersensitive to pressure, ache under light contact, or carry a persistent sense of weight.
  • Easy Bruising: Skin in the affected areas bruises easily.
  • Swelling: Swelling, especially in the legs, is a common symptom.
  • Fat Texture: Fat may feel nodular or fibrotic, like small lumps or bumps under the skin.
  • Fatigue: Many individuals with lipedema experience fatigue.
  • Emotional Impact: Lipedema can significantly impact a person's emotional well-being, causing anxiety, depression, and self-esteem issues.

Is Lipedema Treatable?

Yes. Comprehensive treatments can effectively manage symptoms, halt disease progression, and permanently remove lipedema fat tissue.

How is lipedema treated?

There are two ways to treat lipedema: conservative (without surgery) and surgical (with liposuction). At Dr. Edouard Coeugniet's clinic in Montreal - Canada. He specialized in lipedema reduction (Liposuction) surgery. Liposuction is recommended when other measures are insufficient or if lipedema worsens. Excess fat is suctioned out using special cannulas, taking care to protect the lymphatic vessels.

Lipedema Treatment with Liposuction

in Montreal, Quebec — Canada

Dr. Edouard Coeugniet uses a specialized liposuction technique to address lipedema. It is designed to relieve pressure and congestion caused by the buildup of fatty tissue without damaging nerves and blood vessels.

What Is Lipedema Treatment with Liposuction?

At Lipœdème Montréal, Lipedema treatment with liposuction is a specialized surgical approach led by Dr. Edouard Coeugniet, MSc., to address the unique challenges of Lipedema, a chronic condition causing heavy aching, disproportionate fat accumulation. Unlike traditional liposuction, Dr. Coeugniet employs the most effective cutting-edge surgical techniques for lipedema includes Water-Assisted Liposuction or Body-Jet technology, And other advanced surgical techniques for lipedema liposuction specifically designed to target fibrotic lipedema fat while preserving lymphatic function.

This minimally invasive procedure reduces symptomatic discomfort, improves mobility, and enhances body contour, offering patients lasting relief and improved quality of life. Dr. Coeugniet’s expertise ensures personalized treatment plans, combining liposuction with comprehensive care to achieve optimal outcomes for each patient. Lipedema treatment with liposuction removes this excess fat, relieving significant discomfort and mobility issues.

The procedure

The aim of liposuction for lipedema is to reduce limb bulk, reduce significant discomfort, and to improve mobility and functioning. Liposuction for chronic lipedema can be done under general or local anaesthesia. Several small incisions are made in the limb.

Lipedema reduction surgery - WAL technique

The procedure can take 1 to 4 hours depending on the size of the treatment area. Immediately after liposuction, a compression bandage is applied to the limb to control any bleeding and to prevent postoperative oedema. Antibiotics are typically prescribed as prophylaxis after the operation. When the wounds are healed after the procedure, a custom-made compression garment is worn. This garment is typically revised and refitted multiple times during the first year until the oedema volume has been reduced as much as possible.

Benefits of Lipedema Reduction Surgery (WAL Liposuction)

Lipedema reduction surgery goes far beyond cosmetic improvement; it is a life-altering functional procedure designed to restore your health and mobility. There are many advantages to lipedema treatment with liposuction, including:

  • Significant Pain Reduction: Safely removes the painful, hypersensitive, and inflamed lipedema fat tissue, drastically reducing heavy and aching sensations in the limbs.
  • Restored Mobility: Eliminates the physical bulk of diseased tissue from the inner thighs and calves, resolving gait issues, reducing joint strain, and making walking easier.
  • Halts Disease Progression: Physically extracts abnormal fat cells that cannot be shed through diet or exercise, effectively stopping or slowing the advanced stages of the condition.
  • Protects the Lymphatic System: Decompresses local tissues to improve natural lymphatic circulation, significantly reducing chronic fluid swelling (edema).
  • Enhanced Silhouette and Skin Health: Resolves severe disproportion between the limbs and the torso, while reducing tissue friction, skin irritation, and the risk of localized infections.
  • Improved Quality of Life: Restores physical freedom, boosts body confidence, and allows patients to return to the active lifestyles they enjoy.
  • Minimally invasive procedure with less trauma to the body And Faster recovery compared to traditional liposuction methods

Lipedema Postoperative Care and Recovery Guidelines

A successful recovery depends heavily on your post-surgical care. Following Dr. Coeugniet’s specialized protocol helps reduce swelling, protects your lymphatic system, and ensures optimal aesthetic results. Lipedema reduction surgery (specialized liposuction) recovery typically takes 4 to 6 weeks for daily activities, though final swelling may take up to 6–12 months to subside. Recovery requires strict post-operative care, including continuous compression therapy and manual lymphatic drainage massages, to minimize fluid leakage and promote healthy circulation.

    Essential Recovery Protocols:

  • Compression Therapy: Medical-grade compressionis critical to reduce swelling, minimize bruising, and help skin conform to the new contours. You will likely need to wear these garments 23–24 hours a day for the first 6 weeks. Ensure sleeves or stockings are fitted correctly to prevent fluid pooling or skin bulging.
  • Manual Lymphatic Drainage (MLD): Begin professional MLD sessions within the first week after surgery. This specialized massage stimulates fluid circulation, speeds up healing, and prevents tissue hardness (fibrosis). Routine MLD massages are highly recommended to prevent fluid buildup (seromas) and maintain healthy lymphatic flow.
  • Rest and Mobilization: Start Walking Immediately: Gentle walking begins on the day of surgery. It is crucial for promoting blood circulation and activating the lymphatic system. Refrain from heavy lifting, intense exercise, and swimming for at least 4 to 6 weeks to allow the body to heal. Avoid strenuous exercise or heavy lifting until cleared by Dr. Coeugniet.
  • Nutrition and Hydration: Stay Hydrated: Drink plenty of fluids to assist the body in processing and flushing out excess tumescent fluid. Consume a high-protein, low-sodium diet to support tissue repair and minimize fluid retention.
  • Wound and Scar Care: Keep Incisions Clean. Gently wipe incisions with prescribed cleansers (e.g., hydrogen peroxide) as directed, and allow fluid drainage. Avoid direct sunlight on incision sites to prevent hyperpigmentation.
  • Follow-Up Appointments: Attend all scheduled postoperative visits so Dr. Coeugniet can monitor your healing, check your skin retraction, and adjust your compression requirements.

Risks and Complications of Lipedema Reduction Surgery

While Dr. Edouard Coeugniet utilizes advanced, specialized lymphatic-sparing protocols to maximize patient safety, any advanced surgical procedure carries potential risks. Understanding these complications is an essential step in your patient education journey:

  • Temporary Swelling & Bruising: Mild to moderate fluid retention (edema) and bruising are a standard part of the early healing process.
  • Skin & Contour Irregularities: Areas of uneven skin texture, temporary numbness, or minor asymmetry can occur as the treated tissue settles.
  • Tissue Changes: Some patients may experience localized firmness, temporary tissue fibrosis (scar tissue), or small nodules under the skin during recovery.
  • Skin Laxity: Following high-volume fat removal, some degree of loose skin is common. Advanced options like J-Plasma skin tightening can be integrated to address this.
  • Lymphatic System Stress: Although rare with lymphatic-sparing techniques, damage to superficial lymph vessels can theoretically lead to secondary lymphedema.
  • General Surgical Risks: As with any surgical procedure, there are minimal baseline risks of infection, localized bleeding, or a reaction to anesthesia.

Minimizing Your Risks

At Lipœdème Montréal, Dr. Coeugniet heavily reduces these risks by staging large-volume treatments into smaller, safer sessions and using gentle, highly precise technologies.

Following your strict postoperative guidelines—including continuous medical-grade compression therapy and professional Manual Lymphatic Drainage (MLD)—is equally critical to ensuring a smooth, successful recovery.

Dr. Coeugniet's Understanding of Lipedema: A Focus on Women's Health

At his Montreal / Westmount clinic, Dr. Coeugniet recognizes Lipedema as a distinct and often debilitating chronic disorder of the adipose (fat) and connective tissues, a condition observed almost exclusively in women. This distinct adipose disorder is marked by a disproportionate and frequently hypersensitive buildup of fat cells, primarily affecting the limbs – most commonly the legs and arms – with a striking symmetrical distribution that sets it apart from general weight gain. While less frequent, other areas of the body can also be affected.

A key characteristic Dr. Coeugniet observes is Lipedema's strong correlation with hormonal shifts. The condition frequently emerges or worsens during pivotal periods of hormonal change, such as puberty, pregnancy, or menopause, underscoring the complex interplay of genetics and endocrinology in its development. Crucially, the irregular fat of Lipedema is notoriously resistant to conventional weight loss methods like diet and exercise, a frustrating reality for many patients. While Lipedema is a lifelong condition for which there is no absolute cure, Dr. Coeugniet and his team are dedicated to providing comprehensive management strategies designed to significantly alleviate symptoms, improve comfort, and enhance quality of life. These strategies encompass both conservative therapies and, where appropriate, specialized surgical interventions tailored to the individual's needs.

Lipedema Treatment:

Frequently asked questions (FAQ)

How can I identify if I have lipedema?

Lipedema is primarily recognized by fatty tissue accumulation, particularly in areas like the thighs, buttocks, calves, and upper arms. If the lower half of your body seems disproportionately heavier than the upper body, this could be a sign of lipedema.

How long does lipedema surgery take?

The length of lipedema liposuction surgery depends on number of areas treated, the volume of fat removed and on use of ultrasonic device. It may take between two and four hours to perform the surgery.

How many surgeries are needed for lipedema?

Most lipedema patients require one to two surgeries to sufficiently treat the Lipedema affected areas. The number of surgeries is determined by the patient's body, the amount of excess fat that needs removal, and the type of anesthesia used.

What is the difference between lipedema and lymphedema?

Lipedema and lymphedema are separate conditions, but they can coexist in a condition called lipo-lymphedema. Lipedema is a condition characterized by irregular fat accumulation, primarily in the legs and sometimes arms, that is resistant to diet and exercise. Whereas, lymphedema is a condition characterized by an irregular accumulation of lymph fluid caused by a dysfunction or blockage in the lymphatic system.

Is there a possibility of lipedema recurrence post-surgery?

Liposuction procedure, especially lipedema liposuction, offers a long-term solution and is considered an effective treatment for lipedema. However, in some cases, there might be a mild recurrence over time. Collaborating with Dr. Édouard Coeugniet will ensure you receive the best treatment plans tailored to your needs.

How long does lipedema liposuction last?

Lipedema liposuction typically yields very long-lasting or even permanent outcomes. Most patients remain satisfied with their refined contour for many years. It’s essential to maintain a healthy daily routine to preserve these results.

Which lipedema-affected areas can be treated with liposuction?

Almost all areas with lipedema tissue accumulation can benefit from liposuction. Whether it’s your arms or buttocks for contouring, liposuction can significantly reduce fatty tissue and enhance the affected area’s aesthetics. For example, in addition to those other areas, thigh liposuction is a common procedure to address lipedema on the outer thigh, significantly reducing fatty tissue in that area.

How many liposuction sessions will I need for my lipedema?

Liposuction is designed to permanently eliminate fat cells, alleviating lipedema’s side effects. Immediate improvements can be observed post-surgery, with treated areas visibly reduced.

The number of required sessions varies: some patients achieve the desired results in one, while others might need more. During your initial consultation, you’ll receive a custom-made treatment plan addressing your specific needs.

Does lipedema come back after liposuction?

Long-Lasting Results: Unlike non-surgical treatments that require ongoing maintenance, the results of liposuction for lipedema are long-lasting. Once the fat cells are removed, they do not grow back, although patients are advised to maintain a healthy lifestyle to prevent new fat accumulation.

How successful is liposuction for lipedema?

Results showed significant post-operative improvements in spontaneous discomfort, edema, bruising, mobility, and quality of life among lipedema patients undergoing liposuction. However, over half of the patients still required conservative therapy after surgery.

Can I Combine Lipedema Lipo With Other Surgeries?

While liposuction is often sought for lipedema treatment on its own, it can also be combined with other treatments. For some, it’s a chance to address excess skin in the lipedema-treated areas concurrently.

Others may see it as an opportunity to undergo additional enhancements, such as a tummy tuck, fat transfer, or breast surgery. If you’re considering integrating multiple treatments, talk to Dr. Édouard Coeugniet during your initial consultation.

Tips for Making an Informed Decision

Getting a procedure is a tremendous personal investment. However, it is not a decision that should be made lightly or in a hurry. Rushing might influence you to choose the wrong shape or go too big, which may impair you from running, jumping, or performing certain physical activities with ease. Go too small, and you may feel underwhelmed with your new look.

  • Compare notes or seek others’ opinions It is a good idea to speak with your friends, family, or partner about choosing the correct breast implant for your body and lifestyle. Be open to other people’s opinions and suggestions, especially if they have your best interests at heart.
  • Knowledge is power From a plastic surgeon’s point of view, we recommend learning as much about the specific shapes and sizes, surgery incisions, and types of breast implants on the market. Knowing your reasons also play a role in your decision making.
  • Know your budget Consider the cost of your preferred implant. For example, teardrop-shaped implants are more expensive since they require a textured surface to prevent rotation and a trained surgeon to place them.
  • Seek professional advice It is important to understand that no two breast augmentations are alike. Just because saline implants looked beautiful on your best friend does not mean that the same implant will be the right choice for you. This is why plastic surgeons always recommend face-to-face consultations.
